SampleHQ Request Form

SampleHQ Request Form is a free WordPress plugin designed to help teams manage product sample requests directly from their WordPress sites. It is aimed at modern packaging, marketing, sales, and fulfillment teams seeking an end-to-end solution for organizing, capturing, and processing sample requests without relying on external services or accounts.

The plugin includes a sample library where users can manage products with categories, SKUs, images, descriptions, and custom fields. Products can be added individually or imported in bulk via CSV. The visual form builder offers a drag-and-drop interface with 17 field types, including file upload, address, and consent fields, as well as support for multi-column layouts, conditional logic, and multi-step wizard mode. Forms can be embedded on any page using a Gutenberg block, Elementor widget, or shortcode, with CSS and JavaScript loading only where needed.

A sample picker field allows site visitors to browse products with images, category tabs, and search functionality, and to select samples and specify quantities. The picker supports card grid, checklist, and list view display modes, along with maximum selection counts and quantity input. Submissions are managed through a dashboard that enables viewing, searching, filtering, starring, and exporting submissions as CSV files. Both administrators and submitters receive email notifications for new requests.

The plugin offers integration with WooCommerce, allowing WooCommerce products to be used in the sample picker and adding a "Request a Sample" button to product pages. Security and privacy are emphasized, with features like honeypot, rate limiting, CSRF tokens, server-side MIME validation, protected upload directories, and optional Cloudflare Turnstile for spam protection. All data is stored locally in the WordPress database, with GDPR readiness achieved through consent fields, configurable IP retention, and integration with the WordPress Privacy API. 2 Level AA standards.

While fully standalone, the plugin can optionally connect to the SampleHQ platform for additional workflow features such as fulfillment tracking, CRM integrations with HubSpot and Salesforce, and team collaboration. This makes it a comprehensive tool for organizations managing product sample programs through WordPress.
